How can I get out of something called Chirohealth?

A while back I made a mistake and went to a random chiropractic clinic where i was urged to sign up for something called Chirohealth at a yearly fee. Supposedly this made my payments smaller to the clinic. Well the clinic turned out to just want my money, i watched more videos and had fake backrubs than anything. Now I can't find the brochure for Chirohealth and I have no idea how to get out of this yearly fee that I will never need. Is anyone familiar with this or a phone number for them??
